What to Eat After Piles Surgery: A Day-by-Day Diet for the First Four Weeks

The first motion after piles surgery is the moment every patient dreads. Almost everything you eat in the first week decides whether it is uncomfortable or uneventful.

The short answer

  • Days 1–2: liquids and very soft food dal water, clear soup, thin khichdi, chaas, coconut water, curd, banana, papaya.
  • Days 3–7: soft solids soft rotli, well-cooked dudhi and lauki, moong dal, curd rice, papaya and soaked anjeer.
  • Week 2 onwards: back to a normal thali, rebuilding fibre towards 30 g a day.
  • Start the stool softener on day one do not wait for constipation to appear. This is the single most important instruction on this page.
  • Drink 2–2.5 litres daily. Dehydration after surgery is the commonest cause of a hard, painful first motion.
  • Masala is not the enemy. A small randomised study found resuming a spicy diet after stapled surgery did not worsen recovery.1 Deep-fried food and low fibre are the real problems.

Why the diet after piles surgery is the opposite of most operations

After an abdominal operation we usually want the bowel to rest. After surgery for piles whether laser haemorrhoidoplasty, LigaSure, or stapled haemorrhoidopexy we want the opposite. The operated tissue sits exactly where stool passes, so we want you eating, drinking and passing a soft, formed motion early. A patient who becomes constipated in the first week is the patient who has a difficult recovery.

That is why I start stool softeners on the day of surgery rather than waiting to see what happens. Fibre, fluid and a softener are not optional extras after this operation they are part of the treatment.

Day-by-day diet after piles surgery

Day 1–2

Liquids & very soft

  • Dal water, clear vegetable soup, thin khichdi
  • Chaas (છાશ), coconut water, lemon water
  • Curd, soft banana, papaya (પપૈયું)
  • Prescribed stool softener / isabgol from day one
  • Sitz baths for comfort as advised
Day 3–7

Soft solids

  • Soft rotli, well-cooked dudhi, tinda, lauki
  • Moong dal (મગની દાળ), khichdi, curd rice
  • Papaya, banana, 3 soaked anjeer (અંજીર)
  • 2–2.5 litres of fluid a day
  • Short walks indoors, several times daily
Week 2–4

Back to a full thali

  • Reintroduce bajra/jowar rotla, salads, guava
  • Dal or beans at both main meals
  • Masala and normal home cooking are fine
  • Walk 20–30 minutes daily
Week 4 onwards

Preventing the next one

  • ≈30 g fibre and 8–10 glasses of fluid daily, permanently
  • Two minutes on the toilet, no straining
  • Surgery removed the pile; habits stop the next one forming

The rule that matters most: if you have not passed a motion by day 3, or if it is hard when you do, tell us do not simply push harder or take a stimulant laxative from the chemist. The softener dose can be adjusted in a phone call.

What to eat in week one and what to leave alone

Week one: safe and helpful

  • Khichdi, dalia, curd riceSoft, low residue, easy to finish even with no appetite
  • Moong dal and dal waterProtein for healing without heaviness
  • Well-cooked dudhi, lauki, tinda (દૂધી, ટીંડા)Gentle and hydrating this is the week they genuinely suit
  • Papaya, banana, soaked anjeerSoft fruit that keeps stool moving
  • Curd, chaas, coconut waterHydration you will actually drink
  • Isabgol at bedtime with a full glass of waterBulk without stimulating the bowel

Week one: leave these for later

  • Deep-fried farsan (ગાંઠિયા, ભજિયા)Slows the gut and adds nothing you need right now
  • Large helpings of plain white riceAlmost no fibre when you can least afford a hard stool
  • Rajma, chole, cabbage in the first few daysExcellent fibre, but the gas is uncomfortable so soon after surgery
  • AlcoholDehydrating, and it interacts with painkillers
  • Excess tea and coffeeMore than 3–4 cups works against your fluid intake
  • Stimulant laxatives bought over the counterCramping and urgency are the last things you want this week

Making the first motion easy

  • Take the softener the night before you expect to go.
  • Drink a glass of warm water on waking.
  • Use a footstool so your knees sit above your hips this straightens the anorectal angle and reduces the effort needed.
  • Do not sit and wait. If nothing happens in two minutes, get up and come back later.
  • A warm sitz bath afterwards is a comfort measure many patients find helpful.
  • Some spotting of blood on the paper in the first two weeks is expected. Heavy fresh bleeding, fever, or inability to pass urine is not call us.

Call the clinic straight away if you have: heavy or continuous bleeding, fever above 38°C, severe pain not controlled by your prescribed medication, inability to pass urine, or no bowel motion at all by day 4 despite the softener.

Rebuilding fibre from week two

From about day 10 you can start moving back towards the 30 g of fibre a day that prevents recurrence. Increase gradually roughly 5 g every 3–4 days and raise your water at the same time, or you will get bloating and blame the food.

Frequently asked questions

When can I eat normal food after piles surgery?

Most patients are on soft solids by day three and back to a normal thali by the start of week two. There is no fixed rule about waiting the guide is comfort and stool consistency, not a calendar.

Can I eat spicy food after piles surgery?

Yes, once you are back on normal food. A small randomised study in India (67 patients) found that resuming a spicy diet after stapled haemorrhoidopexy did not worsen recovery. The exception is if you also have an anal fissure, where a randomised trial found chilli does aggravate pain.

When will I pass my first motion, and will it hurt?

Usually between day 2 and day 4. With a softener started on day one, adequate fluid and soft food, most patients describe it as uncomfortable rather than painful. If it has not happened by day 4, call us rather than straining.

Is bleeding after piles surgery normal?

Light spotting on the paper or in the pan for the first two weeks is expected as the operated area heals. Heavy, continuous or fresh bleeding is not contact us the same day.

Should I take isabgol after piles surgery?

Usually yes, and often long term. Take 1 teaspoon to 1 tablespoon in a full glass of water or chaas at bedtime, never dry. It is a bulking fibre rather than a stimulant, so it does not make the bowel dependent.

When can I return to work and normal activity?

Most patients having day-care laser or stapler procedures return to desk work within 3–7 days. Heavy lifting and long driving are better delayed to around two weeks. Individual recovery varies follow the advice given at your discharge.
