Sudden severe lower abdominal pain - particularly when it prevents normal movement, causes collapse, or is the worst pain the patient has experienced - is always a surgical emergency. The most important causes are: perforated appendicitis, ruptured ectopic pregnancy (in women), ovarian torsion, and colonic perforation. All require immediate hospital evaluation. Time is critical.

Is sudden severe lower pain always an emergency? Yes - always needs evaluation. The severity and sudden onset suggest a serious process: perforation, torsion, rupture, or advanced appendicitis.
Most important causes? Perforated appendicitis, ruptured ectopic pregnancy, ovarian torsion, perforated Meckel's diverticulum, and occasionally perforated peptic ulcer with referred lower pain.

For Women: Blood beta-hCG Test Must Be Performed First

Any woman of reproductive age with sudden severe lower abdominal pain must have a blood pregnancy test (beta-hCG) performed immediately - before other investigations. A positive result in the context of severe lower pain means ectopic pregnancy until proven otherwise. Ruptured ectopic pregnancy causes haemorrhage and can be fatal within hours. This test must not be omitted or delayed.

Causes of Sudden Severe Lower Abdominal Pain

Appendicitis (Advanced)

  • Usually preceded by progressive pain over hours
  • Sudden worsening = perforation occurring
  • Brief "false improvement" then generalised pain
  • Rigid abdomen
  • Emergency laparoscopic surgery

Ectopic Pregnancy (Women)

  • Sudden, severe, may radiate to shoulder
  • Missed period + positive pregnancy test
  • Internal haemorrhage
  • Collapse, pallor, low BP
  • EMERGENCY - laparoscopic salpingectomy

Ovarian Torsion (Women)

  • Sudden onset, severe, right or left lower
  • Ovary twists on its blood supply
  • Ischaemia develops rapidly (hours)
  • Ultrasound may show cyst; Doppler reduced flow
  • Emergency laparoscopic detorsion

Other Causes

  • Perforated Meckel's diverticulum
  • Sigmoid volvulus (left lower)
  • Strangulated inguinal hernia
  • Perforated colonic diverticulum
  • Testicular torsion (young men) - scrotal, lower abdomen

Go to hospital immediately if sudden severe lower pain is accompanied by:

  • Collapse or feeling faint - haemorrhage (ectopic rupture)
  • Missed period in a woman - ectopic pregnancy until proven otherwise
  • Rigid abdomen - peritonitis from perforation
  • Pain spreading from right/left lower to whole abdomen
  • Scrotal pain in young men with lower abdominal pain - testicular torsion
  • Unable to walk or stand upright

An ectopic pregnancy occurs when a fertilised egg implants outside the uterus - most commonly in the fallopian tube. The growing pregnancy cannot be sustained in the tube and eventually causes it to rupture, releasing blood into the abdominal cavity. Rupture causes sudden severe lower abdominal pain, shoulder tip pain (from diaphragmatic irritation by blood), and haemorrhage - rapid blood loss leads to collapse and can be fatal without emergency surgery. Any woman of reproductive age with sudden lower abdominal pain and a missed period must have a blood pregnancy test immediately.

Ovarian torsion occurs when the ovary (often with an attached cyst) twists on its supporting ligaments, cutting off its own blood supply. It causes sudden, severe, one-sided lower abdominal pain - often accompanied by nausea. The pain may be intermittent initially as the ovary twists and untwists. If untreated, the ovary undergoes irreversible ischaemic damage within hours. Emergency laparoscopic surgery (detorsion - untwisting the ovary) saves the ovary if performed promptly. The ovary cannot be saved after necrosis. Time is critical for fertility preservation.
