PPIs suppress acid but do not address the underlying cause. If the cause is H. pylori and it has not been eradicated, symptoms recur when PPIs stop. If the diagnosis is functional dyspepsia, PPIs control symptoms but do not fix the disordered gastric physiology. This is why identifying and treating the root cause - not just suppressing acid - is essential for long-term management.

Not exactly. Gastritis is inflammation of the stomach lining - a pathological finding seen on endoscopy or biopsy. Dyspepsia is a symptom complex. Gastritis can cause dyspepsia, but many patients with dyspepsia have no gastritis on biopsy (functional dyspepsia). Conversely, some patients with histological gastritis have no symptoms at all. The relationship is not one-to-one.

Current guidelines recommend that new-onset dyspepsia in patients above age 55 should be investigated with endoscopy to exclude gastric malignancy. Below 55, endoscopy is indicated when alarm features are present (weight loss, bleeding, dysphagia, vomiting, anaemia) or when symptoms persist despite 4-8 weeks of appropriate treatment including H. pylori eradication.

Spicy food does not cause dyspepsia in the sense of initiating the disease. However, capsaicin (the active compound in chillies) can irritate an already inflamed or hypersensitive stomach, worsening existing symptoms. For patients with established dyspepsia, reducing chilli intensity often improves comfort. Moderate spice is generally tolerable - concentrated chilli powder and raw green chillies are the usual aggravators.

Absolutely. NSAIDs (ibuprofen, diclofenac, naproxen, aspirin) are among the most common causes of chronic dyspepsia in India. They inhibit the protective prostaglandins in the stomach lining, leading to erosions, ulcers, and persistent pain. If you take painkillers more than 2-3 times per week, discuss alternatives with your doctor. Paracetamol (acetaminophen) is generally safer for the stomach.

Short-term PPI use (4-8 weeks) is safe and well-established. Long-term use (beyond 8 weeks) is appropriate for specific conditions - GERD with oesophagitis, Barrett's oesophagus, patients on long-term NSAIDs who cannot stop them. For functional dyspepsia, PPIs should be used at the lowest effective dose and periodically reassessed. Long-term concerns include vitamin B12 deficiency, magnesium depletion, and marginally increased fracture risk - all manageable with monitoring.

Visceral hypersensitivity means the sensory nerves in your stomach are abnormally sensitive. A normal amount of food distending the stomach produces pain or discomfort that a person with normal sensitivity would not feel. This is the core mechanism behind functional dyspepsia - particularly the EPS subtype. It explains why the stomach looks normal on endoscopy but still produces significant symptoms. Treatment targets this sensitivity directly - with PPIs, neuromodulators, or both.

Stress is a well-established aggravating factor. The gut-brain axis - the bidirectional communication between the brain and the GI tract - directly affects gastric motility, acid secretion, and visceral sensitivity. Chronic psychological stress slows gastric emptying, impairs accommodation, and lowers the pain threshold in the stomach. Many patients report that dyspepsia worsens during stressful periods and improves during relaxed ones. Addressing stress is a genuine part of treatment, not a dismissal of symptoms.

Test-and-treat means: test for H. pylori first; if positive, treat with eradication therapy; if negative, start a PPI trial. Endoscopy is reserved for those who do not respond. This strategy is recommended for patients under 55 with no alarm features and is particularly cost-effective in India where H. pylori prevalence is high. It avoids unnecessary endoscopy in many patients while treating the most common correctable cause.

Yes. Gallstone-related dyspepsia presents with post-meal upper abdominal fullness, bloating, and discomfort - particularly after fatty foods. It is frequently misdiagnosed as acidity or gastritis in Indian clinical practice. The differentiating features are right upper quadrant predominance, fatty-meal triggers, and sometimes referred pain to the shoulder. An ultrasound identifies gallstones reliably. Laparoscopic cholecystectomy resolves the symptoms.

GERD is primarily an oesophageal condition - acid refluxing from the stomach into the oesophagus causes heartburn (retrosternal burning) and acid regurgitation. Dyspepsia is primarily a gastric condition - pain, burning, and fullness originating from the stomach and duodenum. GERD symptoms rise upward from the chest; dyspepsia symptoms are centred in the upper abdomen. They can coexist in the same patient and may require treatment for both.

The most common reason is antibiotic resistance - particularly to clarithromycin and metronidazole, which are increasingly resistant in Indian H. pylori strains. Incomplete adherence to the 14-day course (stopping early because symptoms improved) is another common reason. Smoking during treatment also reduces eradication rates. Failed first-line therapy requires a different antibiotic combination for the second attempt, ideally guided by local resistance data.

Functional dyspepsia itself does not typically cause significant weight loss. However, patients who eat progressively less to avoid symptoms may lose weight gradually over months. Significant or rapid weight loss in the context of dyspepsia is an alarm feature - it raises concern for gastric malignancy, peptic ulcer disease, or another organic cause and warrants prompt investigation including endoscopy.

No single food cures dyspepsia, but certain foods are better tolerated. Soft, low-fat, non-spicy preparations are generally easiest on a sensitive stomach - plain khichdi, curd rice, steamed vegetables (dudhi, turia), soft roti with mild sabzi, banana, and diluted chaas (buttermilk). Eating these in smaller, more frequent portions is more important than any specific food choice. A 2-week food diary helps identify personal trigger foods to avoid.

See a specialist when: symptoms have persisted for more than 4 weeks despite dietary changes and OTC antacids; you have any alarm features (weight loss, bleeding, difficulty swallowing, persistent vomiting); you are above 55 with new symptoms; you have not been tested for H. pylori; or your dyspepsia is affecting your nutrition, weight, work, or daily life. Self-management with antacids beyond a few weeks delays proper diagnosis.
